To become a Utah Alcohol Training & Education Seminar Provider, complete the application form (adobe pdf format ) and submit the agency's curriculum and all training materials to the Division of Substance Abuse and Mental Health for review and approval. Training curriculums must be in compliance with UT Administrative code R523-23.
